Ways to Give

A true measure of the quality of a community is how its people care for others who need their help.  We’re fortunate to live in an area where giving, generosity and assisting others are deeply held values.  Cloquet Community Memorial Hospital Foundation provides a way for you to support advances in treatment and technology across a spectrum of health care services at Community Memorial Hospital, CMH Raiter Clinic and Sunnyside Health Care Center. Some donors find comfort, others find joy in connecting to the community by giving a gift of hope and healing.

Outright Gifts

Often the easiest to arrange, many donors support us through gifts of cash, checks, credit cards (Visa/MasterCard), securities or stocks, real estate and in-kind gifts.

Life-Income Gifts

One of the following trust options may be the best fit for you:

  • Charitable Remainder Annuity Trust – A trust that pays a set income annually.

  • Charitable Remainder Unitrust – A trust that pays a fixed percentage of the trust’s assets.

  • Charitable Lead Trust – A trust that pays a fixed or variable income to the Cloquet Community Memorial Hospital Foundation for a set term, and then passes to heirs.

Bequests in Wills

Please tell us if you have named the Cloquet Community Memorial Hospital Foundation in your will. We would love the opportunity to share our appreciation. If you are considering this option, an example of bequest language for the Cloquet Community Memorial Hospital Foundation is: “I, (name) give, devise and bequeath to the Cloquet Community Memorial Hospital Foundation (written amount or percentage of the estate or description of property) for its unrestricted (without conditions attached) use and purpose.” Should you like to restrict your bequest, please consult with Cloquet Community Memorial Hospital Foundation when you make your will to be certain your intent can be accomplished.

Life Insurance

You may name Cloquet Community Memorial Hospital as the beneficiary for income tax deductions.

Retirement Plan

By naming Cloquet Community Memorial Hospital as the beneficiary of the remainder of the assets after your lifetime, you can avoid leaving heavily taxed gifts to heirs.

Community Memorial Hospital and Sunnyside Health Care Center enjoys a proud legacy of employee philanthropy and we are honored that so many of our caregivers and their families choose to make Cloquet Community Memorial Hospital one of their annual giving priorities. Through a tax-deductible gift to the CCMH Foundation, CMH’s employees help to ensure that the same quality care they deliver every day will be available to their friends, family and neighbors in their time of need.

Making a Gift

Making a gift to the CCMH Foundation’s Employee Giving Campaign is easy and convenient! Simply select one of the methods below:

  1. To enroll in our simple payroll deduction program, print out the Employee Gold Star Giving Form available here.The form is also available in the Foundation office, and on the CMH SharePoint intranet site.
  2. Submit the signed form to the Foundation office or drop it off at the switchboard of the hospital to be put in the Foundation’s interoffice mail.
  3. To make a one-time employee contribution with your credit card, please use the “Donate Now” button above. You can also make a one-time gift on the Employee Gold Star Giving Form.
