EMPLOYEE GIVING
Community Memorial Hospital and Sunnyside Health Care Center enjoys a proud legacy of employee philanthropy and we are honored that so many of our caregivers and their families choose to make Cloquet Community Memorial Hospital one of their annual giving priorities. Through a tax-deductible gift to the CCMH Foundation, CMH’s employees help to ensure that the same quality care they deliver every day will be available to their friends, family and neighbors in their time of need.
Making a Gift
Making a gift to the CCMH Foundation’s Employee Giving Campaign is easy and convenient! Simply select one of the methods below:
- To enroll in our simple payroll deduction program, print out the Employee Gold Star Giving Form available here.The form is also available in the Foundation office, and on the CMH SharePoint intranet site.
- Submit the signed form to the Foundation office or drop it off at the switchboard of the hospital to be put in the Foundation’s interoffice mail.
- To make a one-time employee contribution with your credit card, please use the “Donate Now” button above. You can also make a one-time gift on the Employee Gold Star Giving Form.
